APPOINTMENTS AND AVAILABILITY:
Appointments can be arranged at your convenience during daytime and evening hours, Monday through Friday.
Full fee is charged for appointments not canceled at least 24-hours in advance. Payment is expected at the time of service.
Our office will be happy to provide assistance necessary to obtain insurance reimbursement.
